OHIP Form

Click on the link below to access the OHIP reimbursement form. OHIP may reimburse for covered scans if the wait times are too long in your area or you can not access a necessary procedure in time.

Given that many of the health care services in Ontario involve long waiting lists, OHIP may not honour your particular request for reimbursement since you accelerated your scan or treatment by going to the US.

There is no guarantee that OHIP will pay for any particular scan or procedure done in the U.S. If you have questions about this, please contact OHIP in advance of booking your procedure or scan to obtain advance approval.

Our general advice is to not assume that OHIP will pay for your treatment or scan but it doesn't hurt to apply for reimbursement in case your situation qualifies.
